Policies & Procedures

This section articulates my policies and procedures regarding fees, payment, insurance coverage, confidentiality and privacy. It also contains important Forms for you to read, download, print and fill out prior to our first appointment. Please don’t hesitate to contact me if you have any questions.

Treatment Sessions & Fees

During our initial phone conversation, a first consultation appointment is arranged and is usually scheduled within 10 days unless you feel it is urgent to see me sooner. Together we then determine future appointments. Fees vary depending on type of service and will be discussed during our initial conversation. Fees for service are payable at each session by cash or check; I do not accept credit cards.

Insurance Coverage

I am an “out of network” insurance provider. Insurance policies do cover my services provided you do not have a restriction to only “in-network” providers and the policy is open to “out of network” provider. A portion of my fees may be reimbursed as part of the “out-of-network coverage” in some insurance plans. Please be sure to give me information about your insurance plan during our initial conversation, so we can determine if and how your insurance may reimburse you for a portion of my fees.

Your psychotherapeutic information cannot be disclosed without your written permission. Exceptions include:

  • If I suspect child abuse or dependent adult or elder abuse, I am required by law to immediately report to the appropriate authorities.
  • If a client is threatening serious bodily harm to another person or persons, I am required to notify the police and the intended victim.
  • If a client intends to harm him or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission, as provided by law, in order to ensure their safety.

Confidentiality & Privacy

The confidentiality and privacy of your psychotherapeutic information is protected by federal and state laws. At the time of your first consultation, a copy of the HIPPA Privacy Policy document will be presented to you for review and acknowledgement.

Cancellation Policy

The dates and times of the therapeutic sessions and consultations are reserved for you in advance.

If you fail to cancel a scheduled appointment at least 24-hours ahead of time, I cannot use that time for another client. You will be charged the full fee for any missed appointments, no-shows or cancellations that are made with less than 24-hour notice, unless you are ill or experiencing a real emergency. Payment for missed sessions will be expected at your next scheduled appointment. Thank you for consideration in regard to this matter.
